KaVo SONICflex retro (surgery): Set of tips for minimally invasive, axial retrograde preparation, filling of the root canal and preparation of undercuts

 

Overview

The problem

The retrograde root canal treatment: limited space, restricted lateral access. Bone substance has to be protected and the stability of the tooth preserved. Rotating instruments sometimes produce the opposite effect: large bone windows, more difficult axial preparation of the canal.

The solution

The KaVo SONICflex retro system (0.571.0381) permits gentle and efficient sound-based retrograde root canal treatment with contra-angle diamond-coated tips:

The bone window can be kept to minimal size, as the tips are very small in size and have a delicate shape. This protects the healthy bone tissue.

SONICflex Tips Consumables

Preparation of the cavity with the smallest cylindrical tip (no. 16/17) to a depth of penetration of at least 3 mm (diamond-coating).

If needed, further gradual processing with larger tip sizes (tip 18/19 = special sizes) until the entire width of the cavity is included in the preparation, i.e. preparation to the dentine with no old root filling material or debris remaining.

 

Special tips make access significantly easier when the root curvature is pronounced (tip no. 55).

 

R - Application of retention

SONICflex Tips Consumables

Preparation of an undercut (retention) with a T-shaped SONICflex retro tip of the largest working size (tip no. 20, 21).

Preparation should proceed without pressure and without contact of the oscillating tip to other instruments (e.g. supporting hooks). This prevents the operational efficiency of the tip from being limited.

The removed dental material and calculus is flushed out of the canal by pump movements during preparation.

The problem

Ideally, approximal adhesive inlays and onlays fit exactly into the cavity: with different bevel angles and edges, harmonious transitions, no undercuts. Practically this is extremely difficult to achieve. Rotating instruments often give rise to the opposite: irregular shapes and edges and hence a poor basis for the dental technician, unnecessarily large extensions.

 

The solution

KaVo SONICflex prep ceram permits perfect finishing and exactly defined cavities through precise transfer of the geometry of the tips directly to the tooth substance. With tips specially developed for adhesive inlays and onlays.

Design of tips: SONICflex prep ceram

For improved control during the achievement of the desired cavity geometries, a sonoabrasive preparation tip has been developed for the adhesive inlay cavity: the SONICflex prep ceram.

Its basic form is trapezoidal and the preparation surface is coated with a 46μm-diamond layer.

The occlusally divergent tip has a bevel angle of 75° cervically and a bevel angle of 60° laterally.

The lateral and cervical surfaces are mutually linked through a pronounced, rounded profile.

Application

 

The SONICflex prep sonoabrasive preparation tips, facilitate the complete avoidance of iatrogenic damage to adjacent teeth, whilst preparing the desired cavity geometry.

With clinically-proven, rotary instruments, old fillings and restorations may be efficiently removed and "basic preparation" performed.

For "fine preparation" of bevelled proximal margins, diamond-coated instruments with oscillating motion are especially suitable.

SONICflex prep ceram tips for preparation of laboratory-created prosthetics, are covered with a fine diamond-coating (grain size 46 μm).

Only two instrument tips are required for thorough treatment of all the teeth:

a tip with a left-deflected (KaVo SONICflex paro tips No. 61) and right-deflected distal end.

 

Therefore in order to improve the cleaning result, the interdental areas of previously palatally cleaned teeth, should also be buccally and palatally/lingually treated, with a horizontally introduced tip, held at an angle.

After the cleaning result has been examined with the help of a syringe spray and a fine probe, SONICflex paro tip No. 62 is used in the same way, but from the opposite side, so that complete processing of the root area is ensured. The straight SONICflex paro tip No. 60 can, if required, be used for additional processing of smooth surfaces.
 

Treatment of the root surface

The use of sonic scalers differs significantly from manual instruments that rely on the entire cutting edge sliding over the root surface: in the latter case, each stroke results in a strip-shaped cleaned area (15). Due to their round cross-section, the SONICflex paro airscaler tips are usually in point-contact with the tooth.

Therefore it must be ensured that the entire surface to be cleaned is covered with careful, overlapping 'snaking' movements, whereby it may be assumed that the most distal one to two millimetres of the tip, is actively involved in the abrasive process.

 

After supra- and subgingival scaling, the accessible roughened tooth surfaces should be carefully polished. The enamel zone lends itself to the use of the KaVo PROPHYflex. On the root surfaces or exposed dentine, polish with a prophy cup and polishing paste. Excellent cleaning results that are gentle on the surface can also be achieved with SONICflex clean brushes (13).

Inserting the SONICflex micro tips

The respective working end is screwed into the SONICflex handpiece using the torque wrench enclosed in the delivery, until it snaps in place audibly. The tip is now securely attached to the handpiece.

Use of the hemispherical tip-form

With the hemispherical instrument, the defect in the enamel can be opened-up without preparatory work by rotary instruments. This procedure is especially recommended for superficial demineralisation.

Hemispherical instruments are ideal for minimally-invasive, primary treatment, whereby depending on the situation, access to the caries can be from occlusal, buccal or lingual/palatal.

If proximal caries is present and if several teeth are being treated simultaneously, the prepared cavity in the adjacent tooth affords ideal access to the defect.

For any non-occlusal access, the functionally important edge ridge can be completely or at least largely retained. The preparation tips with their narrow-waisted shafts, also enable undermining of the enamel.

The problem

Periodontal treatments with graft operations are complicated and physically stressful. The furcation proves to be a weak point: it has only limited access and is difficult to clean.

 

 

The solution

The KaVo SONICflex rootplaner tips for easy processing of even remote or narrow furcation regions. Eases the physically stressful periodontal treatment with graft operation for the dentist and patient:

This graphic shows the horizontal, clinical attachment gain on molars with furcation infection that were processed during flap surgery, either with manual instruments (H), or with diamond-coated SONICflex rootplaner tips (DS).
It can be clearly seen that half a year after operation on the teeth processed with DS, a significantly greater attachment gain was produced that could not be maintained over two years, as the corresponding instruments were not available for the recall.

For furcation processing, an attachment resembling the universal tip with a
 2 mm long and 1 mm thick button and two mirror-image, contra-angled attachments with the same button dimensions, were produced (SONICflex rootplaner tips no.s 24, 25 und 26).

All attachments come with a 25 μm grain diamond coating.
